Learning from Serious Incidents (SIs)

 

It is important that appropriate lessons are learnt from Serious Incidents in screening. 

This page will list below those SIs where cross-programme learning points were identified. A standard format is used so that each programme can determine if the lessons are applicable them and, if so, what actions they need to take or ask local screening services to take.
